General Purpose
The Speech-Language Pathologist (SLP) evaluates and treats patients, communicates with families, physicians and other health team members and maintains documentation of services in the medical records.
Conducts inservices and training for facility staff on an ongoing basis.
Essential Duties
• Evaluate patients within twenty-four (24) hours, or 1 business day of physician referral.
If unable to complete evaluation in 24 hours, documentation in medical record clearly defines cause for delay.
• Develop effective treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Supervise Speech-Language Pathologist Assistants in direct patient care and patient related activities, following state practice act.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed.
• Participate in in-services training program for other staff in the facility.
• Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per Speech and Language Pathologist in the Board State Practice Act and governmental and third party payer requirements.
• Record treatment changes per policy and procedures.
• Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
• Participate in discharge planning.
• Act as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologist and student programs.
• Comply with the Speech Language Pathology and Audiology Bureau State Practice Act and the facility policy and procedures.
• Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.
Supervisory Requirements
Supervises CFY - Speech Clinical Fellows, if applicable, and assists nursing with training of Restorative Aides.

Education and/or Experience
A master's or doctoral degree in SLP and meet either of these requirements:
• Is licensed as an SLP by the state in which they provide services;
OR
• In the case of a person who practices in a state that doesn't license SLPs, they must have:
o Successfully completed 350 clock hours of supervised clinical practicum (or be in the process of accumulating supervised clinical experience);
o Performed at least 9 months of supervised full-time SLP services after obtaining a master's or doctoral degree in SLP or a related field; and
o Successfully completed a national examination in SLP approved by the HHS Secretary.
Proficient in computer skills such as inputting data into programs such as Casamba and Point Click Care.

General Purpose
The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by
your supervisors.
Essential Duties
• Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary.
• Use only authorized abbreviations established by this facility when recording information.
• Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.

General Purpose
A Restorative Nursing Assistant (RNA) provides assigned residents with routine restorative nursing care and services in accordance with the resident's assessment and care plan, as directed by the supervisor.
Essential Duties & Responsibilities
* Assist residents with restorative exercises to improve or maintain mobility, strength, and independence
* Encourage participation in activities and out-of-bed mobility unless medically contraindicated
* Assist with ambulation, transfers, dining, splinting, and range-of-motion activities as directed
* Obtain and accurately record resident weights and report significant changes
* Assist residents with bathing, dressing, toileting, grooming, and other ADLs
* Reposition residents regularly to promote comfort and skin integrity
* Promote personal hygiene and maintain a clean, comfortable environment
* Participate in RNA meetings with Rehabilitation and Nursing Departments
* Complete timely and accurate documentation in PointClickCare
* Observe and report changes in resident condition promptly
* Assist residents with appointments, treatments, meals, and activities
* Apply pressure-relieving devices and implement skin protection measures
* Maintain a positive, respectful, and therapeutic environment for residents

Qualifications
Education & Experience
* High school diploma or equivalent required
* One year of long-term care experience preferred
* Experience with PointClickCare preferred
Licenses & Certifications
* Current California CNA certification required

General Purpose
The Staff Physical Therapist evaluates and treats residents, communicates with families, physicians and other health team members, and maintains documentation of services in the medical records.
Conducts in-services and training for facility staff on an ongoing basis.
Essential Duties
* Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
* Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
* Assist nursing department with training of Restorative Aides.
* Supervise Physical Therapy Assistants in direct resident care and resident related activities, following state practice act.
* Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding progress, goals, and discharge planning.
* Participates in Resident Care c onferences, Utilization Review meetings, and Rehabilitation meetings as needed.
* Provides in-services on training programs for other staff in the facility as needed.
* Complete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.
* Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.
* Educate residents, families, and caregivers on treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention strategies.
* Recommend and facilitate the ordering of necessary durable medical equipment for residents to facilitate independence in mobility; i.e., splints, walkers, canes, elevated toilet seats, and grab bars.
* Must maintain confidentiality of residents' information as well as company information.
* Report any problems with department equipment so that it is maintained in good working order.
* Contribute to quality assurance, performance improvement initiatives, and facility rehabilitation program development as assigned.
* Ability to relate positively, effectively, and appropriately with residents/residents, families, staff, and professional colleagues.
Supervisory Requirements
Supervises PT assistants (PTA), aides, and students.
Qualification
Education and/or Experience
Bachelor's degree in Physical Therapy, prefer graduates of Masters or Doctorate Program in Physical Therapy.
Proficient in computer skills such as inputting data into programs for Net Health and Point Click Care.
Language Skills
Ability to read technical procedures.
Ability to read and comprehend policy and procedure manuals.
Ability to effectively present information and respond to questions from managers, coworkers and families.
