Maximus is seeking a Clinical Review Coordinator - LPN to support the Illinois SALT (Screening Assessment for Long Term Settings) program.
In this role, you will be responsible for performing Level I PASRR (Preadmission Screening and Resident Review) screenings, which include reviewing intake submissions, verifying clinical details, and ensuring documentation accuracy in accordance with program requirements and state guidelines.
This position requires strong attention to detail, organizational skills, and a commitment to upholding clinical integrity and compliance standards.
Additional responsibilities may be assigned by the supervisor to meet program needs.

Position Highlights:
* Utilizes the nursing process to identify and achieve patient goals: assessment, planning, implementation, and evaluation.

* Teaches patient/caregiver in various aspects of health care and disease management appropriate to needs/level of education and understanding and documents continuing needs and levels of patients/caregivers understanding.
