The DDP is an advocate for the individual's wants and needs.
The DDP is responsible for advocating at the individuals at ISP meetings, SIS meetings, etc.
The DDP is a liaison for family members, support coordination, host home providers and the community.
Duties and Responsibilities include:
* Advocate for individual's wants and needs on caseload.
* Responsible for participating in the individual's meetings (ie.
ISP, SIS etc)
* Make face to face contact with assigned individuals on their caseload monthly.
* Monitor the individual's progress toward goals and complete a monthly note in the electronic health record to reflect the progress or lack of progress toward goals.
* Participate with other treatment team members in identifying individual's strengths, obstacles and developing an ISP.
* Monitor the individual's progress towards treatment goals and objectives.
* Communicate with individual, Support Coordination, family members, host home providers, and community resources to ensure that individual's needs are met.
* Ensure supervised staff has a clear understanding of results that are required, and outcomes expected.
* Monthly home visits are completed, and home visit form completed.
* If corrective action plan needs completed for items identified on home visit DDP's are to make sure the home provider has a copy and that CAP items are addressed and completed.
* Monthly DDP review of everyone on caseload must be completed.
* HRST is reviewed and updated monthly as needed.
* Monitor SETHRA accounts and help individuals manage their funds so they don't exceed the $2,000 maximum allowable amount.
* Visit individual while hospitalized to assure their needs are being met.
* All training required must be completed.
* On Call Duties including staffing group home as necessary

MISSIONS
Rattaché(e) à la Responsable du Pôle Assistanat et en coordination avec les autres membres de ce Pôle, l'assistant (H/F) a pour mission de contribuer à apporter assistance et aide à l'ensemble des collaborateurs du service.
PRINCIPALES ACTIVITES

* Organiser l'accueil et l'intégration des nouveaux arrivants
* Être le support des collaborateurs dans leur vie au quotidien en anticipant les besoins et en étant force de proposition sur tout schéma d'amélioration
En lien direct avec les équipes d'une ou plusieurs directions juridiques :
* Organiser, coordonner les réunions, rendez-vous et déplacements
* Assurer le suivi des congés/déplacements/formations des équipes concernées
* Gérer les réceptions et envois de courrier
* Assister les équipes concernées dans la gestion de leur budget et du suivi de leurs factures (usage de l'outil interne MEO)
* Assister les équipes concernées dans le suivi de leur activité (tableaux de bord) et dans l'élaboration de leurs documents (Présentation PWP, Compte-rendu de réunion, etc.)
Profil recherché
De formation BAC+2 Assistanat, vous disposez d' a minima 2 à 3 ans d'expérience professionnelle en entreprise ou cabinet de conseil.
Une parfaite maîtrise du français à l'oral comme à l'écrit est indispensable.
Vous maitrisez parfaitement les outils informatiques et vous possédez un bon niveau d'anglais.
