Salary Range $60-$65/hour (Based on Experience)
Hanford Post Acute is seeking a Part-Time Physical Therapist to join our dedicated rehabilitation team.
This role is ideal for a clinically skilled PT who is passionate about delivering high-quality therapy services in a post-acute care setting.
Responsibilities:
* Evaluate patients within 24 hours or 1 business day of physician referral; document cause for any delay
* Develop individualized treatment plans and obtain physician approval
* Deliver care as outlined in the treatment plan
* Supervise Physical Therapy Assistants in patient care and related activities in compliance with the state practice act
* Communicate with interdisciplinary team regarding patient progress and care planning
* Participate in patient care conferences, utilization reviews, and in-service training
* Maintain accurate and timely documentation of evaluations, daily notes, progress updates, and recertifications in accordance with PT Board and payer requirements
* Instruct caregivers and nursing staff on maintenance programs and post-discharge care
* Recommend and assist in ordering durable medical equipment to enhance patient mobility and independence
* Report and monitor maintenance needs of therapy equipment
* Support nursing with Restorative Aide training
Supervisory Responsibilities:
* Supervise PT assistants, aides, and students as applicable
Qualifications:
* Bachelor's degree in Physical Therapy required; Master's or Doctorate preferred
* Current California PT license in good standing
* CPR certification and ongoing CEUs as required by the state
* Strong documentation skills, preferably with experience in Casamba and PointClickCare
* Experience in SNF or post-acute care preferred
* Ability to work collaboratively and maintain positive relationships with residents, families, and staff
Physical Demands:
* Frequent standing, walking, reaching, and lifting (up to 100 lbs occasionally)
* Occasional stooping, kneeling, or crawling
* Vision requirements include close, color, peripheral, and depth perception
