Supervises the Brookdale at Home agency associated with a Brookdale Senior Housing community.
Responsible for the agencys average monthly service hours provided which is less than 700 hours per month.
Ensures the efficient and effective operation of the Brookdale at Home agency and interdisciplinary team process.Trains and supervises less than 10 Brookdale at Home associates.Provides Brookdale at Home service hours to clients which will be 40% of time each week.Delivers and coordinates clinical services for the agency, if applicable and per state regulations.
