Set Pay Rate: $18.78Shift: 5:00pm - 10:00pmEssential Functions:
* Comply with all Agency policies and procedures and follow contract specifications
* Comply with uniform dress code and personal hygiene standards
* Clean all assigned areas in accordance with the contract specifications
* Clean and disinfect restrooms and washable surfaces in public areas
* Descale showers, toilet bowls, and urinals
* Keep restrooms supplied with required paper products and soap
* Perform routine glass and mirror cleaning
* Scrub tile and wash walls
* Vacuum area rugs and carpets; sweep and mop floors
* Pull trash and recycling and place in proper receptacles
* Dust various surfaces, polish furniture and metal, clean glass tabletops
* Clean and disinfect kitchenettes and break rooms
* Clean and disinfect fitness centers, health centers, and child care centers
* Clean walls, partitions, window blinds, and window sills as required by contract
* Clean corners, crevices, moldings, ledges, door jams, metal strips, and overhead surfaces
* Clean elevators, escalators, and stairwells
* Clean and police building exteriors, garages, and janitor closets
* Keep tools and supplies clean and orderly
* Empty and clean any public ashtrays and urns
* Spot clean, service restrooms, and police trash
* Perform or assist with special restoration work, as required
* Move, arrange, and lift heavy items
* Perform emergency cleaning
* Attend work regularly and report on time
* Attend required meetings and in-service training
* Pass and comply with all building and security requirements and procedures
Secondary Functions:
* Assist with inventory
* Clean and maintain equipment
* Strip, refinish, wax, burnish, buff floors, and clean carpets as required by contract
* Maintain floors including porches and steps at entrances
* Assist with special events by setting up and taking down furniture

Position Summary:
Responsible for completing the annual re-assessment for PACE participants.
Essential Duties & Responsibilities:
* Review Profile, Sales Plan & Medical Record of potential enrollee.
Assess medical records for documented diagnosis, medications, physical and mental status.
* Contact potential enrollee to explain & reinforce the purpose of visit and schedule appointment.
* Visit potential enrollee and gather information to validate medical records and to complete MDS.
* Complete the MDS, sign, and have provider review and co-sign.
Enter in Virtual Gateway.
* Upon collaboration with enrollment team for enrollment date, enter home care referrals into EPIC.
* In accordance with MDS due date log, review medical record of participants that are due one month prior to anniversary date.
* Collaborate with primary nurse to validate information gathered from medical record.
Qualifications and Requirements:
* Graduate of an accredited RN program; BSN preferred.
* Current Massachusetts license in nursing.
* Current BLS certification required.
* Exceptional triage assessment skills.
* Ability to communicate assessment findings efficiently, effectively and accurately in verbal and written formats.
* Bilingual a plus.

* Past or present experience with Country Oven Bakery Engine room equipment, specifically the responsibility of the Country Oven Bakery Ammonia system (specific examples are prior or present- Primary or Secondary engine room mechanic) and be a current member of the Emergency Response Team
* Ability to understand complex verbal and written instructions; ability to read diagrams, sketches, operation manuals, manufacturer's specifications, blueprints, etc.; understand basic math; through understanding of mechanics, electrical equipment, hydraulics/pneumatics, welding, and refrigeration; possess working knowledge of OSHA safety procedures and lock out program; knowledge ordinarily acquired through two years certification from vocational or technical school and 5 years prior maintenance experience.
