Key Responsibilities
As a Physical Therapist at City View Post Acute, you will play a vital role in helping residents regain strength, mobility, and independence.
Clinical Duties Include:
* Conduct comprehensive resident evaluations and develop individualized, evidence-based treatment plans
* Deliver skilled physical therapy services focused on improving mobility, function, and quality of life
* Collaborate closely with nursing staff, Occupational Therapists, Speech Therapists, and interdisciplinary care teams
* Educate residents and families on home exercise programs, mobility safety, and long-term injury prevention
* Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
* Monitor resident progress and adjust treatment approaches as appropriate to support successful outcomes
Qualifications
* Current California Physical Therapist (PT) license or eligibility for licensure
* Strong clinical reasoning and communication skills
