The Staff Physical Therapist evaluates and treats residents, communicates with families, physicians and other health team members, and maintains documentation of services in the medical records.
Conducts in-services and training for facility staff on an ongoing basis.
Essential Duties
* Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
* Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
* Assist nursing department with training of Restorative Aides.
* Supervise Physical Therapy Assistants in direct resident care and resident related activities, following state practice act.

* Must maintain confidentiality of residents' information as well as company information.
* Report any problems with department equipment so that it is maintained in good working order.
* Contribute to quality assurance, performance improvement initiatives, and facility rehabilitation program development as assigned.

General Purpose

We are seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team on a PRN basis.
Working under the supervision of a licensed Occupational Therapist, you will provide skilled therapy services designed to help residents improve function, increase independence, and enhance their quality of life.
Key Responsibilities
* Provide occupational therapy treatments as directed by the supervising Occupational Therapist
* Follow physician orders and individualized treatment plans
* Document daily treatments, weekly progress notes, and resident outcomes accurately and timely
* Monitor resident progress and communicate updates to the Occupational Therapist
* Assist nursing staff with training and implementation of restorative programs
* Participate in interdisciplinary care planning, rehabilitation meetings, and discharge planning activities
* Educate residents, families, and caregivers on maintenance programs and discharge recommendations
* Assist with cleaning and maintenance of therapy equipment and treatment areas
* Report equipment concerns and assist in maintaining a safe therapy environment
* Participate in staff education and in-service training programs
* Ensure compliance with facility policies, state regulations, and payer requirements
* Maintain positive and professional relationships with residents, families, staff, and healthcare providers
Qualifications
Education & Experience
* Graduate of an accredited Occupational Therapy Assistant program
* Experience in skilled nursing, rehabilitation, or long-term care preferred
* Experience with PointClickCare (PCC) and Casamba preferred
* Strong communication, documentation, and organizational skills
Licenses & Certifications
* Current South Carolina Occupational Therapy Assistant (COTA) license in good standing
* Must maintain all required continuing education requirements
* Must remain in good standing with the applicable State Board
