General Purpose
The Staff Physical Therapist evaluates and treats residents, communicates with families, physicians and other health team members, and maintains documentation of services in the medical records.
Conducts in-services and training for facility staff on an ongoing basis.
Essential Duties
* Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
* Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
* Assist nursing department with training of Restorative Aides.
* Supervise Physical Therapy Assistants in direct resident care and resident related activities, following state practice act.
* Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding progress, goals, and discharge planning.
* Participates in Resident Care c onferences, Utilization Review meetings, and Rehabilitation meetings as needed.
* Provides in-services on training programs for other staff in the facility as needed.

Position Summary
We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis.
Under the direction of a licensed Physical Therapist, the PTA will provide skilled therapy services designed to help residents improve mobility, strength, function, and independence while enhancing their quality of life.
Key Responsibilities
* Provide physical therapy treatments as directed by the supervising Physical Therapist
* Follow physician orders and individualized treatment plans
* Document daily treatments, weekly progress notes, and resident outcomes accurately and timely
* Participate in patient care conferences and rehabilitation meetings as needed
* Monitor resident progress and communicate updates to the Physical Therapist
* Assist with cleaning and maintenance of therapy equipment and treatment areas
* Train and assist Restorative Aides in carrying out therapy-related programs
* Communicate effectively with residents, families, nursing staff, and interdisciplinary team members
* Participate in staff education and in-service training programs
* Educate residents, families, and caregivers on maintenance programs and discharge recommendations
* Report equipment concerns and assist in maintaining a safe therapy environment
* Ensure compliance with all facility policies, state regulations, and payer requirements
Qualifications
Education & Experience
* Graduate of an accredited Physical Therapist Assistant program
* Experience in skilled nursing, rehabilitation, or long-term care preferred
* Experience with PointClickCare (PCC) and Casamba preferred
* Strong communication, documentation, and organizational skills
Licenses & Certifications
* Current South Carolina Physical Therapist Assistant (PTA) license in good standing
* Must maintain all required continuing education requirements
* Must remain in good standing with the applicable State Board
