As a Louisville East Post Acute PRN Physical Therapist, you will evaluate and treat Residents throughout the facility in our Pathways TBI, Sub Acute Rehab and LTC Units.
Responsibilities include communicating with families, physicians and other health team members and to maintain documentation of services in medical records.
To observe regulations that govern our facility and as assigned by our Director of Therapy to ensure that the highest degree of quality Resident care is maintained at all times.

Job Description: Evaluate patients within twenty four (24) hours, or 1 business day of physician referral.
If unable to complete evaluation in 24 hours, documentation in medical record clearly defines cause for delay.
Develop effective treatment plan and obtain approval for services from referring physician.
Treat patients per the physician treatment plan.
Assist nursing department with training of Restorative Aides.
Supervise Physical Therapy Assistants in direct patient care and patient related activities, following state practice act.
Communicate with supervisor and other health team members regarding patient progress, problem and plans.
Participate in Patient Care Conferences, Utilization Review meetings and Rehabilitation Conferences as needed.
Participate in in services training program for other staff in the facility.
Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per PT Board State Practice Act and governmental and third party payer requirements.
Record treatment changes per policy and procedures.
Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
Participate in discharge planning.
Recommend and facilitate the ordering of necessary durable medical equipment for patients to facilitate independence in mobility; i.e., braces, walkers, canes, elevated toilet seats and grab bars.
Report any problems with department equipment so that it is maintained in good working order.
Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.
Supervisory Requirements Supervises PT assistants, aides and students.
Qualification Education and/or Experience Bachelor's degree in Physical Therapy, prefer graduates of Masters or Doctorate Program in Physical Therapy.

As the Key Account Manager, you will:
* Manage key performance indicators including net revenues and new product introduction in key hospitals in the US.
* Develop working relationships with all stake holders in Key Accounts including health care providers, staff, lab directors, administrative leaders, etc.
and develop relationships with key electrophysiology internal stake holders in order to use a broad range to drive business.
* Provide expert clinical product and technical assistance and training to physicians and EP Lab Staff on the effective use of Electrophysiology catheter products and systems (e.g., The CARTO® System and appropriate software modules including CARTOMERGE™, CARTOSOUND™and the Stockert RF generator) during case procedures within an assigned geography, and in a manner that leads to meeting and exceeding business goals.

The Surgical Account Manager develops our surgical business and ensures the best clinical outcomes.
Our goal is to establish Heart Recovery as new standard of Care which requires a Heart Team approach.
Key Responsibilities
* Identify opportunities, generate market awareness, and drive adoption of Abiomed surgical product portfolio.
* Open new accounts and built KOL´s
* Educating customers regarding the indications, contra-indications, and technical applications of Abiomed product portfolio.
