Position Summary
As a Peer Navigator in Modesto, California, you will be assisting clients and their families in gaining access to primary preventative and specialty health care services.
You will be providing specific assistance with medication management, housing, counseling, support and advocacy.
Additionally, responsible for engaging, educating, and offering support to individuals and their family in order to successfully connect them to culturally relevant health services including prevention, diagnosis, timely treatment, recovery management, and follow-up.
