The Pediatric Hematology/Oncology Clinical Pharmacy Specialist provides advanced, comprehensive pharmaceutical care to children and adolescents with hematologic and oncologic conditions in the ambulatory setting.
This pharmacist will practice as an integral member of the multidisciplinary care team, partnering closely with physicians, advanced practice providers, nurses, and the pharmacy staff to ensure safe, effective, and evidence ‑ based medication management.
The specialist will support chemotherapy management, optimize supportive care, participate in clinical research and quality improvement initiatives, and serve as a preceptor for pharmacy students and residents.
Essential Functions:
* Review and interpret medication orders; assess for clinical appropriateness, therapeutic goals, adverse effects, contraindications, interactions, and disease ‑ specific considerations.
* Evaluate, monitor, and modify drug therapy for pediatric hematology/oncology patients, including chemotherapy and supportive care.
* Provide comprehensive medication counseling for patients and families, including oral chemotherapy education and adherence support.
* Serve as the clinical liaison between the pediatric oncology practice and the Specialty Pharmacy, collaborating with pharmacy staff and support teams to ensure timely medication access and continuity of care
* Advance standardized, evidence‑based care by developing and updating clinical guidelines, pathways, and EMR‑based decision support.
* Participate in drug therapy consults and pediatric oncology clinical research protocols as needed.
* Support quality assurance and improvement efforts to uphold accreditations and ensure compliance with regulatory and institutional standards.
* Provide drug information and clinical recommendations to pediatric oncology providers, nurses, and other healthcare professionals.
* Precept pharmacy students and residents; contribute to educational programming within the pharmacy department.
* Representing Pharmacy Services on committees and workgroups as assigned.
