This role is responsible for obtaining an accurate medication history by interviewing patients and consulting with their outpatient pharmacies.
Major Accountabilities include:
* Obtain medication histories on all assigned patients during scheduled shifts.

The Social Worker, Regional Manager will serve as a liaison between the operations leadership team, the Divisional Manager, Social Worker & field social workers of the assigned area.
