We are seeking a Social Work Navigator/BSW to support two unique Nemours programs!
This role acts as an internal referral coordinator and liaison to Make-A-Wish to ensure that all patients eligible for a Wish are offered the opportunity.
Functions as the hub between patient/ families, the Nemours Medical Team and the Make-A-Wish local organization.
Attends team meetings, huddles and special events to provide education and awareness of the program resources available.
Follows patients throughout the process and collects relevant data.
Also acts as a navigator and coordinator of psychosocial resources for patients served by the Swank Autism Center including linkage to community-based resources, financial assistance, educational advocacy.
Works directly with the assigned social worker and the medical team.
This position supports patients and families receiving care within Nemours Children's Health by providing linkage to community resources, financial assistance, and positive family experiences.
Responsibilities include but are not limited to:
* Family interviewing and psychosocial assessment
* Coordination of care with internal partners and departments
* Financial resource application completion and tracking
* Maintaining relationships with community partners including psychiatric care facilities
* Collaboration with child welfare partners in DE and neighboring states
