Full-Time | Must be an LSW
Skilled Nursing Facility (SNF) experience preferred

Oakwood Gardens Care Center is seeking a compassionate and resident-centered Social Services Director to support the emotional, social, and care-planning needs of our residents and families.

This role plays a key part in helping individuals navigate transitions, access resources, and maintain the highest quality of life in a skilled nursing setting.
Key Responsibilities


* Conduct psychosocial assessments and create individualized care plans.


* Assist residents and families with admissions, ongoing support, and discharge planning.


* Provide counseling, emotional support, and crisis interventions as needed.


* Participate in interdisciplinary team meetings and care conferences.


* Document all social services interventions per federal, state, and facility regulations.


* Coordinate community services, mental health resources, and outside agencies as needed.


* Advocate for resident rights, dignity, and well-being.

Qualifications


* Licensed Social Worker (LSW) - Required


* SNF or post-acute care experience strongly preferred


* Strong interpersonal, documentation, and organizational skills


* Ability to work collaboratively with residents, families, and interdisciplinary staff
