Speech-Language Pathologist (SLP) - Telehealth

Kern River Transitional Care Center - Bakersfield, CA
Rate: $60-$70 per hour (DOE)
Telehealth / Remote Position

Kern River Transitional Care Center is seeking a skilled and compassionate Speech-Language Pathologist (SLP) to provide high-quality telehealth therapy services to our residents.

The SLP evaluates and treats patients, communicates with families and care teams, and maintains accurate and timely documentation.

This role also includes providing staff training and participating in care planning to support optimal patient outcomes.
General Purpose

The Speech-Language Pathologist evaluates and treats patients via telehealth, communicates with families, physicians, and the interdisciplinary team, and maintains all required documentation.

The SLP also conducts in-services and ongoing training for facility staff as needed.
Essential Duties


* Evaluate patients within 24 hours (1 business day) of physician referral; document delays if evaluation cannot be completed in that timeframe.


* Develop individualized and effective treatment plans and obtain necessary physician approvals.


* Provide treatment consistent with the physician's orders and established therapy plan.


* Supervise Speech-Language Pathologist Assistants (SLPAs) in direct patient care and patient-related activities per state practice act.


* Communicate patient progress, concerns, and goals with the supervisor and interdisciplinary team.


* Participate in Patient Care Conferences, Utilization Review, and Rehabilitation Team meetings as needed.


* Deliver staff in-service training and education programs.


* Complete all evaluations, treatment notes, progress notes, recertifications, and documentation in accordance with state practice act, facility policies, and payer requirements.


* Document treatment changes following facility procedures.


* Provide patient and caregiver instruction for maintenance programs and discharge planning.


* Participate in discharge planning processes.


* Serve as a clinical preceptor for CFY/RFP candidates and SLP students.


* Comply with state Speech-Language Pathology and Audiology practice requirements and facility policies.


* Demonstrate the ability to relate positively and professionally with residents, families, facility staff, and healthcare partners.

Supervisory Responsibilities


* Supervise CFY Speech Clinical Fellows, when applicable.


* Assist nursing staff with training Restorative Aides.

Qualifications

Education & Experience


* Master's or doctoral degree in Speech-Language Pathology.


* Must meet one of the following:
+ Licensed as an SLP in the state of California OR
+ In states without licensure (not applicable to CA), must meet clinical practicum, post-graduate experience, and national exam requirements.
