Hanford Post Acute - Hanford, CA | Starting at $60/hr BOA

Are you passionate about helping patients regain their ability to communicate and swallow safely? Join our dedicated team at Hanford Post Acute, where we provide high-quality, person-centered care in a skilled nursing setting.

We are currently seeking a Speech Language Pathologist (SLP) to evaluate, plan, and implement therapy services to meet the communication and swallowing needs of our residents.

Responsibilities Include:


* Evaluate patients within 24 hours or 1 business day of physician referral (with documentation if delayed)


* Develop and implement personalized treatment plans approved by referring physician


* Provide direct treatment and document patient progress in compliance with state and facility standards


* Supervise SLP Assistants and Clinical Fellows as needed


* Participate in interdisciplinary meetings, care conferences, and in-service training


* Provide caregiver education and discharge planning


* Maintain accurate and timely documentation in systems like Casamba and PointClickCare


* Comply with all applicable laws, regulations, and facility policies

Qualifications:


* Bachelor's degree in Speech-Language Pathology required; Master's or Doctorate preferred


* Active and unencumbered California SLP license


* CPR certification and continuing education per state requirements


* Strong clinical reasoning and interpersonal communication skills


* Experience in SNF setting preferred


* Proficient in electronic documentation systems (e.g., Casamba, PCC)

Physical Demands & Work Environment:


* Frequent standing, walking, reaching, and lifting up to 50 lbs


* Occasional stooping, kneeling, or crouching


* Moderate noise level typical of a healthcare facility
