MDS Nurse (RN or LVN)

Position Type:


* Full-Time


* Rate Range: $120,000 - $150,000 (DOE)

General Purpose:

The primary purpose of this position is to oversee the facility to ensure that assessments are completed per guidelines, with a strong focus on the resident assessment process, including individual assessments, MDS assessments, and the resident plan of care.

Essential Duties:


* Oversee weekly PDPM meetings for skilled patients.


* Track and monitor effective utilization of PDPM tools, including Physician Diagnosis Verification, Admission/Discharge CSR(GG), Weekly CSR, PDPM Weekly Tracking Tool, Point Right.


* Track and monitor QM and QASP performance monthly and quarterly, providing data analysis and clinical recommendations for optimal outcomes.


* Provide PDPM and MDS training to new MDS Coordinators.


* Assess and determine the health status and level of care for all new admissions.


* Ensure the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant changes.


* Communicate the level of care for new residents to all relevant disciplines.


* Facilitate interdisciplinary participation in the MDS completion for each new admission and ensure compliance with regulatory time frames.


* Maintain accurate scheduling for all MDS assessments to ensure proper reference dates throughout the resident's stay.


* Ensure accurate data entry and electronic submission of MDS assessments, verifying electronic submissions and making necessary corrections.


* Schedule and conduct resident care conferences in compliance with state and federal regulations.


* Assist in formulating and revising care plans, ensuring identification and prioritization of residents' present and potential problems, establishing realistic goals, and recommending appropriate nursing interventions.


* Evaluate resident care plans for comprehensiveness and individuality, and assess the achievement of desired outcomes.


* Manage all level of care changes within the facility and notify all departments accordingly.


* Generate necessary forms to document level of acuity and changes, transmitting forms for processing as required by state law.


* Maintain confidentiality of all resident care information, including protected health information.

Supervisory Requirements:

This position includes supervisory responsibilities.

Qualifications:


* Education: Graduation from an Accredited School of Nursing.

Bachelor of Science Degree in Nursing preferred.
