What You'll Do


* Lead and oversee the social services department to ensure residents receive emotional, social, and discharge planning support.


* Partner with residents, families, and staff to create care plans that enhance quality of life.


* Build community partnerships and advocate for residents' rights and well-being.


* Support a resident-centered environment through empathy, communication, and problem solving.

What We're Looking For


* Strong communication and organizational skills.


* Dependable, caring, and motivated to serve the elderly population.


* Prior social services experience in healthcare or skilled nursing (preferred).


* Degree in Social Work or related field (required).
