Roseville Care Center is seeking a skilled and compassionate Physical Therapist to join our collaborative and supportive team! This is an exciting opportunity to make a real difference in the lives of our residents while growing your career in a positive and professional environment.

What You'll Do:


* Evaluate patients promptly (within 24 hours of referral) and design individualized treatment plans.


* Provide therapy in line with physician treatment plans to help patients regain strength, mobility, and independence.


* Supervise and support Physical Therapy Assistants and train Restorative Aides.


* Collaborate with nursing staff, families, and interdisciplinary team members on patient progress and discharge planning.


* Document evaluations, treatment notes, progress notes, and recertifications per PT Board and payer requirements.


* Instruct families and caregivers on maintenance programs to ensure continued success after discharge.


* Participate in care conferences, staff in-services, and team meetings to support holistic resident care.


* Recommend and facilitate durable medical equipment to aid mobility and independence.


* Maintain a safe, organized, and efficient therapy department.
