Physical Therapist
We are seeking a skilled and dedicated Per Diem Physical Therapist to join our dynamic team.
In this role, you will be responsible for assessing, planning, and implementing rehabilitation programs to help our residents regain physical function, reduce pain, and improve mobility.
Responsibilities:
* Conduct comprehensive assessments of residents to determine physical therapy needs.
* Develop individualized treatment plans based on assessment findings and goals.
* Provide direct physical therapy services, including therapeutic exercises, manual therapy, and other modalities.
* Educate residents and their families on the therapeutic process and home exercise programs.
* Collaborate with other healthcare professionals to ensure coordinated and comprehensive care.
* Maintain accurate and timely documentation of resident progress and treatment plans.
* Stay current with best practices and advancements in physical therapy.
Qualifications:
* Valid Physical Therapist license in the state of California.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Flexibility to work per diem hours as needed.
