LVN
* Patient Care:
+ Administer medications and treatments as prescribed by the healthcare provider.
+ Monitor patients' vital signs and report any changes in condition to the RN or physician.
+ Assist in developing and implementing individualized care plans for residents.
+ Provide basic nursing care, including wound care, catheter care, and assisting with daily living activities.
+ Ensure patient comfort and safety by responding promptly to call lights and addressing patient needs.
* Documentation and Communication:
+ Accurately document patient care, observations, and interventions in the medical record.
+ Maintain open and effective communication with patients, families, and the healthcare team.
+ Participate in shift handoffs to ensure continuity of care.
* Team Collaboration:
+ Work collaboratively with RNs, CNAs, and other healthcare professionals to deliver comprehensive care.
+ Assist with the training and orientation of new staff members as needed.
+ Participate in care conferences and contribute to the interdisciplinary team's decision-making process.
* Compliance and Safety:
+ Adhere to all facility policies, procedures, and regulations, including those related to infection control and resident rights.
+ Ensure proper use of medical equipment and report any malfunctions to appropriate personnel.
+ Maintain a clean, safe, and organized work environment.
Qualifications:
* Education: Graduate of an accredited LVN program.
* Licensure: Current and valid LVN license in the state of California.
* Experience: Prior experience in a skilled nursing or post-acute care facility is preferred.
* Certifications: BLS (Basic Life Support) certification required.
* Skills:
+ Strong clinical and critical thinking skills.
+ Excellent communication and interpersonal skills.
+ Ability to work effectively in a fast-paced environment.
+ Proficiency in electronic medical records (EMR) systems is a plus.
Physical Requirements:
* Ability to lift and move patients as required.
* Prolonged periods of standing and walking.
* Ability to perform physically demanding tasks, such as repositioning patients.
