Sonoma Post Acute in Sonoma, CA, is seeking a passionate and motivated Assistant Director of Rehabilitation to join our team.

We are looking for either a Physical Therapist Assistant (PTA) or a Certified Occupational Therapy Assistant (COTA) to help lead our rehabilitation department.

Key Responsibilities:


* Assist the Director of Rehabilitation in overseeing day-to-day operations of the rehabilitation department.


* Lead therapy sessions and work directly with residents to achieve their rehabilitation goals.


* Ensure compliance with all regulatory requirements and maintain documentation standards.


* Collaborate with the interdisciplinary team to provide coordinated care.


* Mentor and support fellow therapists and therapy assistants to ensure high-quality care.

Qualifications:


* Licensed Physical Therapist Assistant (PTA) or Certified Occupational Therapy Assistant (COTA) in the state of California.


* Previous experience in a skilled nursing or rehabilitation setting is preferred.


* Strong leadership, communication, and organizational skills.


* A passion for patient care and improving rehabilitation outcomes.
