Napa Post Acute is seeking a skilled and experienced MDS Coordinator (LVN or RN) to join our dedicated team in Napa, CA.

Position: MDS Coordinator

Salary Range: LVN - $90,000-$100,000, RN- $110,000-$120,000

Responsibilities:


* Conduct comprehensive MDS assessments in compliance with state and federal regulations.


* Develop and update individualized care plans based on thorough assessments and interdisciplinary team input.


* Ensure accurate and timely documentation to support optimal reimbursement and compliance.


* Collaborate with nursing staff, therapists, and other healthcare professionals to deliver exceptional resident care.


* Participate in care plan meetings and quality improvement initiatives to enhance resident outcomes.


* Monitor MDS processes to ensure consistency, accuracy, and adherence to all regulatory requirements.

Qualifications:


* Current California LVN or RN license.


* Proven experience as an MDS Coordinator in a skilled nursing or long-term care setting.


* In-depth knowledge of MDS 3.0, care planning, and regulatory guidelines.


* Strong organizational skills with a keen attention to detail.


* Excellent communication and teamwork abilities.
