Temporary Community Health Worker/Promotore I/II

ABOUT THE POSITION

The County of Riverside Temporary Assignment Program is currently seeking to fill multiple Temporary Community Health Worker/Promotore I/II for the Riverside University Health System - Public Health Department throughout Riverside County.

The position is a 5/40 schedule - Monday though Friday - 8:00 a.m.

to 5:00 p.m.

depending on assignment, evenings and weekends may be required.

Candidates with a Community Health Worker Certification are encouraged to apply.

Current Opportunities:


* Indio Community Health Center
+ Incumbent will work as essential member of an interdisciplinary team to perform a wide range of duties associated with community-based, high-touch, person-centered services, provided primarily through in-person interactions.

Incumbent will engage in delivering health care information and services to patients, which include intake services, referrals, health education and outreach, and aid clinical and other professional staff, within established guidelines and policies.


* Riverside University Health System - Public Health, HIV/STD Program
+ Incumbent will help promote the HIV/STD Program's mission to promote healthy sexual behavior and reduce the impact of HIV/AIDs and Sexually Transmitted Diseases (STDs) in Riverside County.

Incumbent will also promote behavior that prevents the spread of HIV and STDs and will provide health care information and services to patients suffering from HIV/STD, which include intake services, referrals, health education and outreach.


* Riverside University Health System - Public Health, Disease Control
+ Incumbent will help provide a variety of services to the community, including preventive care and investigations to prevent the spread of certain communicable diseases within Riverside County.

Incumbent will also provide health care information and services to individuals exposed to a communicable disease, which include intake services, referrals, health education and outreach.

Under general supervision, serves as a vital link between the public and health services within the County of Riverside to promote health among individuals, families, and groups including those who lack access to adequate care, the underserved, and at-risk populations; works as part of a designated interdisciplinary team to ensure high-quality and continuous care is accessible to all program participants; performs other related duties as required.

The Community Health Worker/Promotore II is the journey level class in the Community Health Worker/Promotore series and reports to an appropriate supervisory or manager level position.

Incumbents assist in the implementation of a wide array of activities, including Basic Population Health Management (BPHM) - related interventions, such as wellness and prevention.
