Service Coordinator - HCS
Description
The Harris Center for Mental Health and IDD ("The HARRIS CENTER") is looking for a Service Coordinator to join our team.
This person is responsible for providing assistance to individuals with intellectual disabilities in gaining access to services and supports utilizing a person directed planning process to achieve their desired outcomes.
Services and supports may include waiver programs
medical
psychiatric
residential
financial
and a wide array of social services in the community.
Provides service monitoring through contacts with consumers
Legally Authorized Representatives
program providers and collaterals.
The ideal candidate will display compassion
education
proficiency and experience while assisting the patient.
This critical role assists in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.
What you will do:
* Complete all assessments as specified per department policy and procedural guidelines
* Based on consumer needs
monitor client at a minimum of a quarterly basis
* Update consumer files as needed
* All documentation is completed within established timelines
* Complies with all safety
health
and training requirements
What qualifications you will need:
* Bachelor's degree (B.A.) from four-year accredited college or university with a major in a social
behavioral
or human service field
including but not limited to: psychology
social work
medicine
nursing
rehabilitation
counseling
sociology
human development
gerontology
educational psychology
education
and criminal justice.
* Must maintain a valid Texas Driver's License and be insurable under Agency policy.
* Minimum of one year experience in the human services field.
* Experience working with persons with intellectual disabilities preferred.
* Must possess effective written
verbal communication
and organizational skills.
* Must be able to work autonomously in a fast paced environment.
* Effectively communicates and works with consumers
family members
visitors and fellow employees in a manner that demonstrates knowledge of cultural diversities.
