JOB SUMMARY
The Med Care Manager/QMAP must administer, pour, pass, and document all routinely prescribed medications per physician orders under the direction of the Wellness Director and as approved by the State Medication Administration Regulations.
You will quickly learn how each resident prefers to take their medication.
Some may even play a game of hide and seek, pressing you to find where they may be in the community when it’s time for their medication.

POSITION OVERVIEW
* Administer and document regularly prescribed medications per the Medication Administration record observing the five rights for safe administering of meds and per physician orders.
* Order meds from the pharmacy when and as prescribed, 7 days prior to running out of meds, and as requested by the Wellness Director to ensure that residents always have medications available.
Ensure these medications arrive, if not notify the Wellness Director within 24 hours.
* Immediately after administering medication to a resident, document in the resident’s medical record those medications given.
Date and initial the bubble pack after taking the medication from it.
* Observe and report to the licensed nurse reactions and side effects of medication administered to the residents.
* Take and record vital signs prior to administration of medications which could affect or change vital signs as ordered by the Physician or Wellness Director.
* Administer PRN medications, per physician order.
* Required to report any medication error to the Wellness Director immediately.

The Mobility Assessment Evaluator (MAE) is responsible for reviewing ADA paratransit application materials, supplemental intake and professional verification forms, conducting in-person interviews for applicants, and performing functional assessments with applicants to determine eligibility for ADA paratransit service.
The Mobility Assessment Evaluator (MAE) is also responsible for maintaining accurate documentation and providing final eligibility determinations to the Client.
This position will sit our Honolulu, HI office.
Schedule is Monday – Friday 8am-5pm.
What You’ll Do:
* Determines final eligibility based upon the application, interview, functional assessment and any supplemental information
* Conduct in-person interviews to assess mobility capabilities and review any supplemental information provided
* Determine what functional assessments the applicant will be required to undergo
* Provide written eligibility determination documentation to Client within ADA required and contracted time frame
* Reads and interprets ADA paratransit applications, intake forms, and professional verification forms
* Review interview results and application paperwork and follow up on assessments and professional verifications, as needed
* Maintain ongoing communication with the Quality Assurance team and the management team in regards to, violation of contractual deadlines and/or possible liquidated damages
* Assist Applicants with completing incomplete documents, scan ADA applications, interview guides and other documents into the Applicant’s case, as needed
* Provide further written documentation to Client upon request
* Assist the leadership team in identifying process improvement opportunities
* Conduct functional physical and/or cognitive assessment with applicants that may include:

Position Summary:
The Integrated Health Care clinic is currently seeking NYS Licensed and Permit holding therapists for several local area school districts. The therapist will provide assessments and psychotherapy to students and families.
Liberty Resources Integrated Health Care is the largest provider of outpatient mental health services in Central New York, utilizing a number of evidence-based practices as well as individual, family, group therapy; co-occurring substance abuse and mental health treatment; psychiatric evaluation; and medication management services to adults, children, and families.
We seek a diverse group of staff including LMSW, LCSW, LCSW-R, LMHC and LMFT clinicians.
Liberty also has an onsite pharmacy that enhances clinical outcomes for patients and affords a higher level of treatment integration by having access to Pharmacists as a part of the integrated care team.

Position Overview:
The Therapy Tech has the opportunity to directly interact with patients to assist under the direction of physical and occupational therapists with patient treatment sessions and recovery as well as opportunity for minimal office support work with patient reporting assistance.
Responsibilities:
* Escorts patients, guest, clients, and residents to the private treatment rooms, acute care, or gym areas.
* Assists patients on/off exercise tables or equipment, or in/out of whirlpools.
Provides general supervision/assistance with transfers, gait, and exercise. All the above are at the discretion of the therapist/assistant.
* Prepares whirlpool baths, and dressings, and assists with dressing changes.
Adheres to current infection control practices, including cleaning of whirlpool equipment.
* Applies hot or cold packs under Therapist/Assistant direction.
Prepares patients, guest, clients, and residents using proper positioning and draping, prior to equipment use including ultrasound, electrical stimulation, UV irradiation and therapeutic exercise equipment under therapist direction.
* Monitors/reports response of patients to treatment and informs appropriate supervisor.
Recognizes emergency procedures and reacts appropriately.
* Ensures that linens are changed on treatment tables, and that supplies and equipment are returned to the proper location in a timely manner.
* Identifies malfunctioning equipment and documents needed for repair/maintenance, notifying Supervisor as appropriate.
* Assists with maintaining clean, orderly, and safe work environment.
Follows schedule of cleaning per department policy.
* Assists Administrative Secretary with department paperwork upon request.
This is a secondary duty and should be done so as not to conflict with primary responsibilities.
