The County of Riverside - Riverside University Health System-Public Health is recruiting a Registered Nurse for the Maternal, Child & Adolescent Health/ Black Infant Health Program located in Riverside, Ca.
The incumbent will be responsible for:
* Providing case management services to pregnant and parenting women and their children.
* Conducting health assessments, assisting families with accessing resources and providing information and education.
* May assist with facilitating groups and conducting outreach activities.
* Making home visits to support the needs of the families.
* Complete Health & Developmental Screenings.
* Working with pregnant and parenting at risk mothers that identify as African American.
