PURPOSE AND SCOPE:
Provides nutritional services for the facility’s in-center and home patients as applicable in order to maximize the patient’s nutritional status and improve clinical outcomes.
Functions as a member of the interdisciplinary healthcare team, supporting the FMCNA commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities.
Actively participates in process improvement activities that enhance the likelihood that patients will achieve their individualized patient-specific goals as determined by the patient’s physician.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
* Assesses patient’s knowledge of diet and kidney disease and provides education appropriate to patient’s learning needs regarding the relationship of diet and kidney disease and the impact on the treatment process.
* Provides patient specific detailed education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process.
* Completes nutritional assessment and participates in the interdisciplinary comprehensive assessments.
* Participates with the interdisciplinary team in the development of a written, individualized comprehensive plan of care that specifies the services necessary to address the patient’s needs, in accordance with established Federal and, where applicable, state licensure guidelines.
* Calculates diet prescription according to standard of practice.
* Completes progress notes on all chronic dialysis patients according to established FMCNA guidelines.
Maintains complete and accurate documentation of all appropriate information in medical record according to established FMCNA policies and Federal and state regulations.
* Provides patient with ongoing nutrition assessment and outcome-oriented nutrition counseling necessary to assist patient in achieving and sustaining an effective nutritional status.
* Identifies malnourished patients as well as patients at risk for malnutrition and works collaboratively with interdisciplinary team to identify appropriate interventions, resources or solutions.
Reviews each patient’s albumin level and body weight at least monthly.
Implements plan of care with involvement of patient/family/caregiver.
* Monitors adherence and response to nutrition therapy, adjusts therapy as needed to achieve and sustain an effective nutritional status, documents why the patient was unable to achieve the goals and implements the plan of care to address issues impacting the patient’s ability to achieve the goals.

Responsibilities

Provides Customer Service and Support Services for WIC Nutritional Program:
• Assists the WNA’s with WIC clients by obtaining and accurately recording of height and weight measurements, dietary intakes, and other necessary medical and/or nutritional information.
• Compares laboratory test results, anthropometric measures and/or dietary intakes to establish standards to screen for nutrition risk.
• Identifies clients with special nutritional needs and refers them to the R.D./Nutritionist/WNA for nutrition counseling and development of Nutritional Care Plan.
• Provides basic health, nutrition and breastfeeding information to individual clients and client groups.
• Maintains and updates client documentation through WIC WISE.
• Explains program eligibility requirements and clients’ rights and obligations.
• Provides triage at WIC sites by answering central telephone line, and providing customer service to WIC clients.
• Informs clients about the benefits, services of the WIC program and family planning.
• Instructs clients on prescribed food packages and WIC card benefits and purchasing procedures.
Evaluate clients understanding of this process.
• Conducts monthly group classes and other nutrition education activities as requested by the Center Manager.
• Participates in community outreach efforts as assigned by the Center Manager.
• Attends full staff and in-service training sessions.
• Integrates referrals between WIC and Family Planning, documents referrals to other social and community services.

1) Assist patients/residents with Daily Living Activities such as bathing, dressing, grooming, eating, transfer, ambulation, toileting and other resident needs.
2) Ensure the following tasks are completed on a daily basis; beds are made; trash has been removed from the room, that meals are served to residents and maintain a high level of hygiene in the room.
3) Taking vital signs and reporting observations and any change in condition to the nurse.
4) Practice Infection Prevention and Control measures in compliance with Federal, State and Facility requirements.
5) Document in CNA ECS flow-sheet timely, accurately and completely.
6) Make routine and frequent rounds and ensure those safety precautions/equipment are in place and in working order.
7) Avoid development of skin problems or skin breakdown by providing timely incontinent care and frequent repositioning residents that need assistance.
8) Assist the Activities Department with activities for the residents.
CNA/MED TECH ESSENTIAL QUALIFICATIONS:
Education: Must be certified nurse’s aide in accordance with laws of Florida.
