Department:
Strategic Planning & Development

Under the direction of the Director of Enterprise Population Health, the Population Health Coordinator will support the integration and implementation of new programs and initiatives across Salinas Valley Health.

The responsibilities include assisting with project management, monitoring, reporting and analytics.

The coordinator will support the department by organizing disparate efforts towards common goals and deliverables.

They will ensure work plans are well developed, deliverable timelines are managed, and communications reach key stakeholders.



* Supports various Population Health Management initiatives, including program development, implementation, evaluation, monitoring and reporting.



* Conducts routine research for the department to support project initiatives and stay up to date with new government regulations and payment models.



* Performs ongoing monitoring of health services activities to evaluate effectiveness and determine opportunities for improvement.



* Monitors project status, identifying and resolving/escalating issues and risks, delivering progress reports on projects, and communicating updates to project stakeholders.



* Ensures compliance with contract and program requirements to include compiling data and results.



* Leads smaller, time limited projects, as needed.



* Administers operational dashboards for physician productivity and contract tracking.



* Identifies program development opportunities, organizing internal resources to complete and monitor population health efforts of the organization.



* Serves as support role to the Director, Enterprise Population Health, accomplishing key tasks as assigned.



* Supports healthcare system's population health initiatives.



* Performs other duties as assigned.

Education: Bachelor's degree required.

Licensure: None.

Experience: Minimum of two (2) years' experience in healthcare.

Experience working on population health performance and project coordination or an equivalent combination of education and experience.

The hourly rate for this position is $38.51 - $48.13 The range displayed on this job posting reflects the target for new hire salaries for this position.
