Must be registered with the Board of Behavioral Sciences as an Associate Clinical Social Worker (ACSW), Associate Marriage & Family Therapist (AMFT), or Associate Professional Clinical Counselor

Social Service Practitioner -Psych Testing PCLS (Psychiatric Clinical Liaison Service) consist of Social Service Practitioners who assist with medical psychiatric medical care management, psychosocial/crisis counseling support to patients, and care transition planning into the community.

The PCLS program operates 7 days a week.

Primary support is by the PCLS-SW (SSP) staff to provide behavioral health support throughout the RUHS -Medical Center.

The PCLS-SW (SSP) is requested to assist with Weekend & Float support.

LPS 5150 Certification and Oversight in Riverside County is needed for this position; if a candidate does not currently possess this it will need to be obtained during orientation.

This position will:


* Assists with advisements/notification of the legal status and provides patient rights material, Firearms Prohibition Notification when a patient's legal hold of a 5150/5558/4011.6, or 5250 hearing notification/advisement is required.


* Conduct psychosocial assessments for the team to evaluate patient's mental status, social supports, mental health history, drug history and discharge planning needs.


* Obtaining collateral information, with patient's consent, from family/friends that patient gives or community agencies and placements to assist with a patient's discharge planning needs.


* Communicate with medical treating team and unit staff to ensure proper care & discharge planning for patient's mental health needs.


* Provide documentation in EPIC of consultation recommendations, treatment interventions, discharge planning and follow up.

Schedule is Monday through Friday 8am to 4:30pm.

The Riverside University Health System Medical Center in Moreno Valley is seeking Social Services Practitioner III's to work as a Medical Social Worker.

Under general direction the position will perform difficult social service casework dealing with complex individual and family problems or social service casework for patients and their families in healthcare facilities; plans, organizes, implements, and evaluates all clinical services provided to pediatric patients and needs of children in required assignments; may participate in and assist with conducting in-service training programs; may provide leadership as a program consultant in the utilization of casework concepts and methodology; and performs other related duties as required.
The Social Services Practitioner III is a broad range journey to advanced level classification of the professionally trained social caseworker in the Social Services Practitioner series.
